We want to welcome you to Greenleaf Health & Wellness.  Our mission is to get you so healthy that you no longer need us.  We do this by getting down to the root of the problem so you can flourish.  We utilize a combination of traditional medicine with cutting-edge alternative and holistic therapies.

Practice and Consultative Services

We are a consultative practice aimed at complimenting your primary healthcare practitioner.    It is advised that you have a primary physician. 

We provide services during office hours and appointments only. We do not offer after-hours services.

Any urgent conditions should be addressed through your primary physician, specialist, urgent care center, or emergency room.   Please notify us after you have seen a primary care service. 



The preferred form of communication is through email.  You may contact us by emailing  A response will be provided as soon as possible. You may also leave a message on our voicemail at 866-758-2357.  We do not actively man this number but try to get back to you as we are available.



Because of our practice's unique nature, we are out of network with insurance; however, we are more than happy to provide you with an invoice to submit to insurance.  Please notify us if you would like this document and 48 hours to prepare it for you.  Understand that we do not know what your insurance will cover, and we do not make any claims guaranteeing insurance reimbursement.  Please discuss coverage with your insurance plans.   It is our understanding that Medicare and Medicaid do not accept out of networking bills.


Hours of Operation

Hours of operation are by appointment only and are typically in the office Mondays and Fridays.

Telemedicine appointments are by appointment.  Please check our availability schedule.




Our scheduling system is available to you 24 hours a day through our website.  Please be advised that online is the preferred way to book an appointment. If you are unsure which type of appointment to book, then either book a general wellness appointment or send us your question at


Deposit and Costs

Our prices can be viewed through our scheduling software.  We reserve the right to change the fees, discount services, set up package treatments, and raise/lower prices based on product availability and pricing.

The price you book your appointment will remain constant in that the booking price is the fixed price at that time and will not be changed on you.  For example, if you book during a special or before a price elevation, you get your appointment at the price you booked.

A credit card is required at the time of booking to hold your reservation. 


Phone Calls and Consultations

Please be advised that phone conversations in excess of 5 minutes may invoke a phone consultation fee.   Phone consultation fees will be charged at $5.00 per minute.  An invoice will be sent to you for payment.


No Show & Cancellation Policies

Please cancel and/or reschedule your appointment 48 hours or more before the appointment.  If the cancellation occurs greater than 48 hours  prior, any money paid for a visit can be applied to a newly scheduled appointment.  Cancellations of less than 48 hours notice may incur a charge up to 50% of your visit cost. Any remaining money paid can be applied to your next visit.

No shows can incur a no-show fee of up to 100% of your visit, and the money is forfeited; therefore, it cannot be applied to another appointment. 

No shows of more than three times may result in being excused from the practice.

All of our packages, auto-pays, and gift certificates are not transferable to other clients and cannot be combined with other discounted offers. Also, we do not offer cash refunds for our packages, auto-pays, or gift certificates; however, we will honor an equal exchange (credit) for other services within our facilities as long as it does not violate our no show/cancellation policy


Client Relationships

We value our relationships and are looking forward to working with you as a team.  As in life, not all personalities match up.  If we feel that the client relationship is not beneficial or that we cannot help you, we will be clear and upfront about this.  We wish you the best on your health journey, and we understand that sometimes clients are best served elsewhere. If this is the case, you will receive notification in writing with 30 days notice.  Notice fewer than 30 days can occur in extreme cases, such as in the threat of physical or emotional harm.  In return, if you chose not to follow up, please notify us in writing within 30 days so that we may close your account. 




Legally medical records are the property of the health practice.  However, you are more than welcome to request a copy of your health records. 

Records sent to another physician are sent as a courtesy to you and do not incur an additional fee.

Printed records are $1.00 per page or $100 for the entire chart, whichever is less. New Jersey Law governs this price.

Records sent by email or fax are $0.50 per page or $100 for the entire history, whichever is less.

As per New Jersey state law, records will be saved for seven years, at which time they may be purged without notification to the patient.



We partner with many products and services that we find to be beneficial to our clients.  Affiliate relationships may result in us receiving a fee or paying a fee.  These fees are at no cost to you.  We choose our partners carefully and are only looking to join up with companies that provide value to our overall health goals.  When we suggest products from a partner company, we do so out of the belief that their product or service may benefit you.  We would not risk our relationship by being swayed by financial gain.  Any fees we receive from affiliate relationships are nominal and are invested back into the company to provide you with better services.

We do not take responsibility for or guarantee any of the affiliate services or products.  Consumers are encouraged to use their judgment when utilizing these products. Any complaints should be directed to the product or service companies used.

Any recommended companies and services are just a recommendation.  You are not required to purchase goods or services from suggested companies.


Supplements and Nutraceuticals

The Food and Drug Administration does not evaluate supplements and nutraceuticals. Therefore these products are not intended to diagnose, treat, cure, or prevent any disease.

Because the FDA does not regulate supplements, there can be a wide range of efficacy amongst products. When possible, we recommend medical or pharmaceutical grade products, which we believe have better health value to consumers.
